An unannounced on-site monitoring inspection was conducted on 03/14/2024. There were 63 children, ages 3 months-5 years, and 13 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, lunch and nap. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: playground area with fall zones. The inspector arrived at 9:20 AM and departed at 12:00 PM.

Standard #: 8VAC20-780-140-A Description: Based on record review and interviews, the center failed to ensure that each child shall
have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Evidence:

Standard #: 8VAC20-780-140-B Description: Based on record review and interviews, the center failed to ensure that if the child has had a physical examination prior to attendance, it shall be within the required time frame for the age of the child.
Evidence:
The record for Child # 3 contained documentation that the first date of attendance for was 06/05/2023 and DOB is 06/19/2022/ On the day of the inspection the physical examination was dated 08/19/2022. Standard #: 8VAC20-780-60-A-8 Description: Based on review of children?s records, the center failed to ensure that a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Standard #: 8VAC20-780-330-D Description: Based on observation of the playground area, the center failed to ensure all immediate fall zones had the required amount of resilient surfacing.
Evidence:
Observed on the playground there was no measurable amount of resilient surfacing directly in the fall zones of the slides. According to the attachment VII in the Standards for Licensed Child Day Center the amount of resilient surfacing is six inches based on the height if the equipment observed on the playground.
