Comments:
An unannounced Monitoring Inspection took place on April 10, 2024, between the hours of 12:00pm and 1:00pm. There was a total of 25 children with 3 staff within the supervision guidelines. The children were observed playing on the outside playground. A complete inspection of the physical plant, 5 children and 3 staff records, drill log, policies, emergency supplies and medication was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice. Description: Based on observation, the center did not ensure that the current license was posted in a conspicuous place on the licensed premises.
Evidence:
1) The new license was not posted. The license posted was dated April 11, 2023.

Plan of Correction: The new license has been posted.
