A monitoring inspection was initiated on April 2, 2024 from 2:47 pm until 4:23 pm and concluded on April 15, 2024 after a children/staff records reviewed. There were 35 children present, ranging in ages from infancy to school age, with 7 staff supervising. The inspector completed observations of the children in care transitioning from napping, eating a snack, returning into the facility from a field trip to a local bowling alley, completing handwashing/bathroom breaks, interacting with peers and staff, sitting in high chair and infant equipment, and parent pick up. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 children records and 3 staff records were reviewed.

Standard #: 8VAC20-780-280-B Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence: In the Fours classroom, the laundry room door was unlocked containing multiple laundry detergents with the labels "keep out of reach of children" "caution".Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-320-B Description: Based on observation, and interview with staff, the center did not ensure each restroom area provided for children have toilets that are flushable.
Evidence:
1. In the Fours bathroom area, two of the toilets were not flushing and the water pressure was low during the inspection.

Standard #: 8VAC20-780-340-C Description: Based on observation and interviews, the center did not ensure there always shall be on the premises and on field trips when one or more children are present on staff member who meets the qualifications of program leader or program director and an immediately available staff member, volunteer or other employee, with direct means for communication.
Evidence: 1. On April 2, 2024, there was one staff observed with 18 children returning to the center from the field trip.

Standard #: 8VAC20-780-530-C Description: Based on observation, record, and interview, the center did not ensure there shall be at least two staff members who meet the requirements of subsection A of this section present on the premises during the center's hours of operation, on fieldtrips and whatever children are in care.
