An unannounced monitoring inspection was conducted on 04/12/2024 in response to a self-reported incident. The inspector interviewed 3 staff members and reviewed 1 injury record during the investigation. The inspector arrived at 9:50 am and departed at 10:30 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-290-A-3 Description: Based on staff interviews, the center failed to ensure that in areas used by children of preschool age or younger, electrical outlets shall have protective covers.
Evidence: On 04/10/2024 the preschool classes went into The Hart Gym to play prior to dismissal. At approximately 2:56 pm Staff 3 observed Child 1 (5 years old) walking to the wall. Child 1 then stuck a metal hair barrette into an uncovered electrical outlet. Staff 3 stated that they heard a pop and observed a spark from the electrical outlet. Staff 2 stated that they heard a pop. Child 1 was immediately checked for injury and taken to the school nurse. Child 1 did have some black soot on their right index finger but did not appear to be injured. There were a total of 14 children, ages 3 years-5 years, present in the gym when the electrical outlet was uncovered and the incident occurred.Plan of Correction: Reviewed the standard with the staff. Will ensure that the outlets are covered in all areas used by the preschool classes. Will have extra outlet covers in backpacks to ensure that any uncovered outlets can be covered before the preschool classes enter the room.
