Comments:
A renewal inspection was conducted on April 15, 2024 from approximately 10:40 AM to 1:20 PM. There were 29 children present in the center, ranging in ages from infant to preschool, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records, three staff records, and three board member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names, list staff by positions only.

Description: Repeat Violation
Based on a review of children's records, the center did not ensure that each child's record contained documentation of immunizations required by the State Board of Health before the child can attend the center.

Evidence:
The record for child #2 (enrolled: 06/02/22) did not have documentation of immunizations until 4/25/2023.

Description: Based on a review of children's records and interview, the center did not ensure to obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
1. Administration identified child A who had emergency medications for a diagnosed food allergy. The child's record did not contain a current written allergy care plan. The last allergy care plan was dated 10/11/2021. Child A has current medications and medication authorizations to treat an allergic reaction.

Description: Repeat Violation
Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
In Room #2, one area of the wall had peeling paint and drywall showing at the child?s level. Children in this room are infant and toddlers age.
