Comments:
An unannounced inspection was initiated on 4/10/2024 in response to a complaint that was received by the licensing department on 3/28/2024 regarding allegations in the areas of supervision and building maintenance. The inspection was initiated on 4/10/2024 and completed on 4/16/2024. On 4/10/2024, the inspector was on site from approximately 12:23 pm to 1:26 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Additional documentation was reviewed remotely.

The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. During the investigation an additional violation not related to the complaint was found. See the violation notice for more details.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on interview, the center did not ensure to follow the required rest period ratio of children to staff.

Evidence:
1. In interview, staff stated that they were supervising up to 25 preschool aged children in single ratio for time periods ranging from a few minutes to 30 minutes. The permitted rest period ratio for preschool aged students is 1 staff per 20 children.
2. Administration acknowledged that the classroom was not operating within ratio during these times.

management while she is at lunch.

Description: Based on observation and interview, the center did not ensure that during the designated rest period, cots had linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges as required.

Evidence:
1. During the designated rest period, 15 children were observed without the bottom cover on their cots.
2. Administration acknowledged that the required linens were not available.

Plan of Correction: A notice was posted on the front door to remind parents to bring in a sheet for their child. Parents also donated sheets to the center to be sure mats and cots are covered. Individual notices were sent out as well.
