Comments:
An unannounced monitoring inspection was conducted on 4/15/24 from 9:40am to 12:00pm with program management. There were 105 children in care, ranging in age from three-months-old to five-years-old, supervised by 21 staff. The children were observed playing on the playground, participating in circle time, playing in structured centers, and having their diapers changed and hands washed. Five child records and four staff records were reviewed. The Center has 19 staff with current certification in CPR and First Aid, and 28 staff trained in Daily Health Observation. A sample of six medications and authorization forms were reviewed, and the center has 10 staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 11/16/23 and the most recent Health Inspection on file was dated 1/5/24. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 4/22/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of four staff records, the center did not obtain the results of a criminal history record check from each state that a staff member resided in within the past five years, prior to employment. Evidence: The record for Staff #2, with an employment date of 4/1/24, contained documentation that the staff member had resided in Kentucky within the past five years. There was no criminal history record check from Kentucky on record for Staff #2.

Plan of Correction: Prior to employment of Staff #2, the center requested background checks of Staff #2 from Virginia and Kentucky. The center received the required child abuse/neglect background from Kentucky and the criminal background check from Virginia; both of which met VDOE requirements. However, the center was not aware that the criminal background check portion from Kentucky was not included, and submittal of such request had to be done separately. Upon notice at this inspection, an appointment was scheduled for Staff #2 to complete the criminal background portion on 5/6/2024. The center recorded in writing the licensor's guidance on how to determine another state's process for releasing criminal history records, all of which vary from state to state.

Standard #: 8VAC20-780-510-L
Description: REPEAT VIOLATION Based on observation, the center did not ensure that medications, except for those prescriptions designated otherwise by written physician?s order, were kept in a locked place using a safe locking method that prevented access by children. Evidence: On 4/15/24, in the ?PreK? class, a child?s emergency inhaler was observed on a high shelf in an unlocked cabinet.

Plan of Correction: The medication in question was on a high shelf within a closed-door cabinet, out of reach of children. Upon notice during this inspection, the Director returned the medication to the locked closet. The school held a staff meeting on 4/17/2024, in which we reviewed the proper policy and procedure for storing and locking up medications.
