Comments:
An unannounced renewal inspection was conducted beginning at 12:25 p.m. and ending at 4:00 p.m. with the director. There were 77 children in care ranging in age from 4 months through 5 years old. Classrooms, supplies and materials were observed. Infants were observed being fed, during rest and play time. Two, three and four year olds were observed during nap. Toddlers were observed during nap and afternoon snack. The outdoor play area was observed.

Five staff records and nine children's records were reviewed. Seven medications and authorizations were reviewed.

Information gathered during the inspection determined non-compliance with an applicable standard or law and a violation was documented on the violation notice issued to the program.
Please complete "Plan of Correction" and "Date to be Corrected" cited on the Violation Notice and return a signed and dated copy to the licensing inspector by 5pm on April 24, 2024.

Description: Based on observation, the center failed to ensure that resilient surfacing was adequate in use and fall zones around play equipment. Evidence: On the playground, in the use and fall zones of the composite climber and slide that is approximately four feet high dirt was observed in various areas in the mulch used for resilient surfacing.
