Comments:

An unannounced monitoring inspection was conducted on 4/16/24 from 9:05am to 12:00pm. Children were observed participating in various activities including circle time, music and movement, center time and art. Records were reviewed for five children and six staff members. Medication, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit meeting.

Standard #:	8VAC20-780-60-A
Description:	8VAC20-780-60 Based on record review the center did not ensure that records for each child enrolled contain all the required information. Evidence: 1.The records for Child #1 (date of enrollment 2/26/24) and Child #5 (date of enrollment 2/19/24) did not contain the father's work address and phone number.
Plan of Correction:	The parent's work information will be obtained and put in the record.
Standard #:	8VAC20-780-270-A
Description:	8VAC20-780-270 Based on observation and interview the center did not ensure that areas and equipment of the center are maintained in a safe condition. Evidence: 1. In the infant/toddler room there was radio wire hanging down from a shelf attached to a heavy radio. 2. The hanging wire was in reach of the children in care and posed a strangulation and tripping hazard. 3. Toddler were observed playing near the wire, also posing a tipping hazard.
Plan of Correction:	The wire was secured during the inspection.
Standard #:	8VAC20-780-290-A-3
Description:	Based on observation and interview the center did not ensure that in areas used by children of preschool age or younger, all electrical outlets have protective covers. Evidence: 1.There were protective covers missing from one outlet in the pre-k classroom and one in the hallway. a. The outlets were in children?s reach and accessible to the children in care. 2. Staff confirmed that the outlets were missing protective covers.
Plan of Correction:	Staff will be reminded to check the electrical outlets in their classrooms to ensure they are capped and admin will ensure hallway outlets are capped.

Description:	Based on observation and interview the center did not ensure that medication is labeled with the child's name and maintained in the original, labeled container. Evidence: 1. In the toddler room there was a non-prescription medication was not in the original box and was not labeled with the child?s name. a.The child was in care at the time of the inspection. 2.Staff in the classroom confirmed that the medication was not in the original box or labeled with the child?s name.
Plan of Correction:	The medication will be returned to the parent. Only medication in the original box will be accepted.
Standard #:	8VAC20-780-570-C
Description:	Based on observation the center did not ensure that the record of each child on formula contain the brand of formula used. Evidence: 1.The center did not have a record of the type formula each infant drank. 2. Infant staff confirmed they did not have documentation of the type of formula each infant drinks.
Plan of Correction:	The infant room will begin keeping a record of the type of formula each infant is on.
