Comments:
An unannounced monitoring inspection was conducted on 04/16/2024 from 10:50 a.m. to 1:20 p.m. There were 53 children present, ranging in ages from three to five years old with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: REPEAT VIOLATION
Based on review of records and interview, the center failed to maintain a separate record for each child which contained the required information.
Evidence:
1. Child #1?s record was missing the annual update, first day of attendance and proof of birth.
2. Child #2?s record was missing an address for one of the emergency contacts.
3. Child #3?s record was missing the annual update, first day of attendance, proof of birth and the phone number for one parent?s place of employment.

Plan of Correction: 1. Program staff will update child records to include the listed items.
2. Plan to monitor all child files for accuracy.

Standard #: 8VAC20-780-330-B
Description: REPEAT VIOLATION
Based on measurement and observation, the center failed to ensure that when playground equipment is provided, resilient surfacing complies with the minimum safety standards and that fall zones are free of hazards .
Evidence:
1. On the back playground, the mulch measured four inches by the spiral slide, five inches between the climber and slide and five inches by the purple dinosaur rider. The mulch did not meet the minimum requirement of six inches.
2. The climber in the front play area had a mulch barrier and bare ground within the fall zone on the side of the climber with stairs and on the front side of the climber with the slide there was additional equipment within the fall zone.

Plan of Correction: 1. Mulch is being ordered to replenish the mulch in the rear playground.
2. The additional equipment from the front playground will be relocated to the rear playground. The large climber in the front playground will be moved to meet the fall zone requirements.
