Comments:
An unannounced monitoring inspection was conducted with the Administrative staff from 11 am to 12:50 pm. There were twenty eight children present with 7 staff supervising. The ages of the children ranged from 10 months to 5 years. The younger children were eating lunch, which was brought from home. The children in the Pre-K classroom were engaged in group activities, floor activities and sand table play. Five medications were reviewed as well as five allergy care plans. Five children and six staff files were reviewed. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-270-A
Description: The center shall be maintained in a clean, safe and operable condition
A phone cord to the wall mounted phone was in reach of children in the toddler room. The cord poses a strangulation hazard.

Plan of Correction: The phone cord was moved out of reach of children.

Standard #: 8VAC20-780-510-P
Description: When authorization for medication expires, the medication shall be picked up within 14 days by the parents or disposed of by the center or the parent must renew the authorization.
The parent and physician authorization had expired on 3/1/24 for Acetaminophen for child A . The medication remained at the center and had not been renewed, or returned to the parent or disposed of.

Plan of Correction: We will have the parent sign a new authorization form for 10 days until we can obtain a doctor's long term authorization.
