Updated Sworn Disclosure statement (2/2024) should be used moving forward from today's visit.

Comments:
An unannounced monitoring inspection was conducted today. All rooms were in ratio during the inspection.
Children were observed finishing lunch ,washing hands, listening to stories, and preparing for rest time.
Emergency evacuation drills, cot spacing, top and Bottom covers for resting children, bathroom supplies and first aid supplies were all in compliance today.
1 staff and 3 child records were reviewed today.

Standard #: 8VAC20-780-60-A-8
Description: Repeat Violation
Based on a review of records for allergy action plans, the center did not have a written allergy action plan for a child with diagnosed food allergies, and could not follow a written action plan for another child.
Evidence:
1. Child #1's written care plan called for Zyrtec to be on-site and administered in case of a
exposure to a documented allergen.
2. Child #2 had no written allergy action plan on site.

Plan of Correction: Parent has submitted the FARE form that same day.
Parent brought medication the same day during pick-up.
