*Third request for documentation that the Fire Marshal's office has been contacted regarding the installation of safety gates between rooms.
*The use of an apple corer in CH1
*Sex Offender Documentation - screen shots should be signed and dated by the person completing the search, as it happens. (Not a violation today)
*Staff interactions with children.

Comments:
An unannounced monitoring inspection was conducted today.
All rooms were in ratio during the inspection.
Children were observed self-serving snack, working independently in both Children's House rooms, reading books, working with water and cleaning.
5 child and 2 staff records were reviewed. See Violation Notice for non-compliances cited.

Standard #: 22.1-289.035-B-4
Description: Repeat Violation
Systemic Deficiency
Based on a review of records, staff records did not contain documentation of completed out of
state background checks for residences outside of Virginia within the past 5 years, in the required time frames.
Evidence:
1. Staff A's record did not have a completed Criminal History Search result on file, for a
residence outside of Virginia. prior to employment. (Date of Hire (DOH): 2/7/24).
2. Staff B's record did not have a competed Child Abuse and Neglect search on file for a
residence outside of Virginia, before 45 days of employment had expired. The search results (email) did not have a date, or the person's name that was searched on the document that was on file. (Staff B - DOH: 4/8/21)
3. Staff A's record did not have a completed Child Abuse and Neglect search on file for a residence outside of Virginia, before 45 days of employment had expired. (DOH:2/7/24).
The letter on file, had no name of the person being searched, and was for a different LLC.

Description: Based on a review of records, one child record did not contain a written allergy action plan for a diagnosed food allergy.
Evidence:
Child #1's record listed a dairy allergy. There is no written allergy action plan including physician's instructions on file.

Description: Based on observation the facility did not maintain the center, inside and outside, in a clean, safe and operable condition.
Evidence:
The playground fence, near the CH 1 room, had:
1. A support pole with broken brackets and missing bolts.
2. The end cap is missing on the cross bar at the top of the fence posing a finger entrapment
area.
3. The cross bar at the top of the fence was attached to the support pole with a piece of green fabric.
4. 3 staff coffee cups containing hot beverage were observed sitting on a low shelf outside the CH1 bathroom.

Description: Based on observation, hazardous materials were not kept in a locked place using a safe locking method to prevent access by children.
Evidence:
1. CH1 room had a wet jet mop with cleaning solution installed leaning against the children's bathroom wall.
2. Lysol wipes were sitting on top of the mirror in the children's bathroom.
3. A spray bottle of bleach was sitting on top of a shelf in the CH 1 room.

Description: Based on observation, outlets were uncapped in a classroom.
Evidence:
The CH2 room had a power strip in use, accessible to children with multiple outlets uncapped.

Based observation, the center did not always keep children under 10 years of age within actual sight and sound supervision of staff.
Evidence:
In the Children's House 2 room, multiple children aged 3 -6 years old were working on rugs behind a shelf unit out of sight and sound of staff.

Description: Based on observation, children's hands were not washed before and after eating.
Evidence:
1. A child was observed working with playdough, then eating their snack. Hands were not washed before eating snack.
2. Two children were observed eating snack, then starting an individual activity without first washing their hands.

Description: Based on a review of sunscreen containers, the containers were not labeled.
Evidence:
There was no name on 2 sunscreen containers in the Children's House 2 room.

Description: Based on a review of diaper creams, all required information was not documented.
Evidence:
1. One container was not labeled to show ownership.
2. Per the authorization form available, the form was good for one year. The form expired 2/2024. (Child 2)
3. There were no records available that documented the child's name, date of use, frequency of application and any adverse reactions for any of the diaper creams present.

Standard #: 8VAC20-780-560-F
Description: Based on observation, the center provided snack menu was incomplete.
Evidence:
1. The menu posted is not dated. It shows "week 1"; however, the is no date to identify the dates for week one.
2. Substitutions were not posted for the menu. Tuesday listed muffins for snack. No muffins were provided for snack. Bananas, apples, applesauce, cucumbers, crackers and oranges were observed being eaten.
