Technical Assistance:
It was discussed with the program director that the labeling of spray bottles with "disinfectant" should indicate the actual contents (brand) in the bottle.

Comments:
A monitoring inspection was initiated and concluded on 04/16/2024. The inspector arrived at 9:45 AM and departed at 11"50 PM. There were 49 children present, ranging in ages from infant to 5 years, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:
Standard #: 8VAC20-780-500-B
Description: Based upon observation, the facility has not ensured that disposable diapers are disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal and the diapering surface is cleaned with soap and at least room temperature water and sanitized after each use.
Evidence:
1. Diapering was observed in the two year old classroom. The lids on the diaper disposal systems were not connected. Staff was using her hands to move the lid to dispose of diaper changing waste.
2. Staff in the two year old room sanitized the changing table with disinfectant but did not first wash the changing surface with soap and water.

Plan of Correction: The facility responded with the following:
All classrooms have spray bottles of both soap and water and disinfectant. Diaper changing procedures will be reviewed with all staff at this week's staff meeting. Lids on the step-down diaper cans will be connected at all times.

Standard #: 8VAC20-780-510-P
Description: Based upon review of medication stored for potential administration to children, the facility has not ensured that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.
Evidence:
There was ibuprofen stored for potential administration to child 2. The parent authorization indicated 2/1/24 to 2/11/25. The parent authorization was not renewed after ten days and the ibuprofen was not returned to the parent.

Plan of Correction: The facility responded with the following:
The medication was returned to the parent during the inspection. In the future we will ensure that parent authorizations are for no longer than ten days and all medication is returned to the parent when the authorization expires.
