Comments:

An unannounced monitoring inspection was conducted on April 18th, 2024. Two inspectors were on site from approximately 9:45am until 1:30pm. There were 28 children present with 7 staff supervising. There was a total of four classrooms visited, ranging in ages from infants to preschool. Children were observed in variety of activities including interacting with staff while doing art activities, playing with building blocks, reading, singing songs, coloring, eating lunch. Inspectors interacted with staff and children in care. If you have questions regarding this inspection, please contact the Licensing inspector. The inspectors reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and six staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Description: Based on record review, the center did not ensure to obtain documentation that each child received immunizations required by the State Board of Health before the first day of attendance. Description:	Description: Based on observation, staff failed to wash their hands before and after a diaper change and to ensure children's hands were washed after toileting. Evidence: 1. Staff #6 was observed putting on gloves to change an infant diaper without washing hands, changed the infant diaper, and did not wash hands after diapering the child. 2. In the infant room, the hands of two children were not washed after their diapers were changed.
