Technical Assistance:

Discussed medication authorization form versus allergy action plan, handwashing, injury reports , orientation documentation and Program Leader Qualification documentation.

Comments:

An unannounced monitoring inspection was conducted on April 17, 2024 from 11:30pm- 1:50 pm. The inspection concluded on April 17, 2024. At the time of entrance, 43 children were in care with 10 staff members present. Additional staff was present to assist. The sample size consisted of 5 children records and 5 staff records. Children were transitioning from lunch to nap time. Outside areas were inspected during the inspection. Areas of non-compliance are identified on the violation notice.

Standard #:	22.1-289.035-B-4
Description:	Based on record review and staff interview, the licensee did not ensure to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. Evidence: -Staff #1 lived in NC and Hawaii in the past 5 years. -Staff #1 hire date was 3/04/2024. -Staff #1 record did not obtain a sex offender check results for NC and Hawaii or documentation for Child Abuse and Neglect background check was submit within the 30 days from date of employment for NC and Hawaii.
Plan of Correction:	The center will submit all out of state background checks today.
Standard #:	8VAC20-780-160-C
Description:	Based on record review and staff interview, the licensee did not ensure at least every two year from the date of initial TB screening the staff get update TB test. Evidence: Staff #5 most recent TB test was 12/2021 more than 2 years.
Plan of Correction:	Told staff to get updated TB test.
Standard #:	8VAC20-780-40-M
Description:	Based on record review, the licensee did not ensure the date was on a current allergy list. Evidence: The allergy list reviewed in multiple classrooms did not have a date.
Plan of Correction:	Allergy list will be dated today.

Description:	Based on observation and staff interview, the licensee did not ensure to have a working battery operated radio. Evidence: The radio was not working during the inspection.
Plan of Correction:	The center will buy a new radio.

Description:	Based on record review and staff interview, the licensee did not ensure shelter in place procedures were practiced twice a year. Evidence: During 2023 there was one shelter practice drilled practiced confirmed by drill log.
Plan of Correction:	Sending reminder to myself to do two shelter in place for 2024.

Description:	Based on observation and staff interview, the licensee did not ensure the lunches were labeled with name and date. Evidence: There were 2 lunch bags labeled with names but no date in the early preschool classroom.
Plan of Correction:	The center will talk to the staff and parents to remind them to put the name and date on the lunches.
