Comments:
An unannounced monitoring inspection was conducted on 4/10/2024 from 11:00am to 2:00pm. At the time of the inspection there were 40 children in care and 10 staff members. Children were observed participating in various activities including manipulatives, outdoor play, lunch and nap time. Records were reviewed for 5 children and 5 staff members. Evacuation drills, emergency supplies and other required records and postings were also reviewed. There was no medication on site during this inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation
Based on observation and interviews, it was determined that the facility did not ensure all areas and equipment of the center shall be maintained in a safe and operable condition.
1.There was a frayed carpet that posed a tripping hazard in the three-year-old classroom.
2.There was a broken and splintering wood edging on the 1?2-year-old playground.
3.On the preschool playground there was rust on the handlebars and pedals of a tricycle.

Description: Based on observation and interviews, it was determined that the facility did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
1.There were cleaning and disinfectant containers of Clorox bleach and Odor Ban in an unlocked shed on the playground.
2.Staff #1 confirmed the shed was not locked.

Description: Based on observation and interviews, it was determined the center did not ensure that disposable diapers are disposed of in a system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence:
The foot lever on the diaper pail in the infant room did not lift the diaper pail lid. Staff had to lift the lid with their hand to throw away a diaper.
