Comments:
An unannounced monitoring inspection was conducted on 4/2/24 from 10:45am to 12:45pm. At the time of the inspection there were 25 children in care and 5 staff members. Children were observed participating in group activities and preparing for lunch. Records were reviewed including seven children?s records and six staff records. Posting requirements, medication and emergency supplies were reviewed during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review and interviews, it was determined the center did not ensure that documentation was obtained that each child in care had received immunizations as required by the State Board of Health before the child attends the center.
Evidence:
1.The records for child #1 and child #2 did not contain documentation of immunizations.
2.Staff #4 confirmed the immunization documentation was not in the records.

Standard #: 8VAC20-780-60-A
Description: Based on review of children?s records and interviews, it was determined that the center did not ensure that each child?s record contains all required information.
Evidence:
1.The records for child #1 and child #2 did not contain parents name, address, phone number, parent workplace and work phone number, two emergency contacts, school currently and previously attended, allergies or intolerances, signed written agreements, and documentation of viewing proof of identity.
2.The record for child #1 did not contain the name and phone number of the child?s physician.
3.Staff # 4 confirmed the required information was not in the records.

Standard #: 8VAC20-780-190-A-3
Description: Based on a review of staff records, it was determined the center did not ensure the program director met all the qualifications required under 8VAC20-780-190-A-3.
Evidence:
1.Staff #4 stated she was the director.
2.The record for staff #4 did not contain documentation of 12 semester hours or 18 quarter hours of college credit in a child related field.
3.The record for staff #4 contained documentation in a letter dated 2/9/2024, that an allowable variance would be requested to allow the staff member time to complete the program director qualifications.
4.As of the date of the inspection, 4/2/2024 an allowable variance request was not received by the Office of Child Care Health and Safety.
