Comments:
An unannounced on-site monitoring inspection was conducted on 04/24/2024; a staff record review will be conducted on a later date. There were 68 children, ages 5 years-11 years, and a total of 8 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staffing and supervision, programs, emergencies and nutrition. The children were observed arriving to the program, having an afternoon snack, going outside to play, and participating in homework time. A total of 7 children?s records and 5 medications were reviewed. The inspector arrived to the center at 2:45 pm and departed at 4:10 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-510-G
Description: Based on medication review and staff interviews, the center failed to ensure that medication shall be labeled with the child?s name, the name of the medication and the dosage amount, and the time or times to be given.

Evidence: Medication 4 was located in a Ziplock baggie and was not labeled with child?s name, the dosage amount, or the time or times to be given. Staff 1 stated that the medication belonged to Child 2.

Plan of Correction: The medication was found in the child's backpack and was sent home with the child's parent today.

Standard #: 8VAC20-780-510-P
Description: REPEAT VIOLATION

Based on document review and observation, the center failed to ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

Evidence: The medication authorization form for Medication 5 expired on 09/12/2023. Medication 5 was observed on-site during the 04/24/2024 inspection.

Plan of Correction: The medication will be sent home with a note for the parent to call before the child can return to care.
