Comments:
Today an unannounced Renewal Inspection took place between the hours of 11:10am-12:30pm. There were 3 classrooms observed with a total of 18 children with 7 staff within the supervision guidelines. The children were observed being held/fed bottles, playing with toys and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, 1 out of 2 staff did not submit documentation of a negative tuberculosis screening.

Evidence:
1. Staff #2 (doh: 3/25/2024) did not have documentation on file of a negative tuberculosis screening at the time of the inspection.

Plan of Correction: We will have the employee get their TB screening as soon as possible and make sure that moving forward, all staff obtain their TB screening within the required timeframe.

Standard #: 8VAC20-780-70
Description: Based on a review of staff records, 1 out of 2 staff records reviewed did not have the required records kept for each staff person.

Evidence:
1. Staff #2 did not have documentation of 2 out of 2 required references of character and reputation at the time of the inspection.

Plan of Correction: We will get the staff references for the employee as soon as possible and moving forward we will get the references done prior to hire as required.

Standard #: 8VAC20-780-570-A
Description: Based on observation, when a child is placed in an infant seat, the belt was not fastened securely.

Evidence:
1. Child #1 was observed sitting in an infant bouncy seat and the buckle was not fastened securely at the time of the inspection.

Plan of Correction: The was corrected during the inspection. We will make sure all infants/children in any type of seat have their buckle securely fastened moving forward.

Standard #: 8VAC20-780-570-E
Description: Based on observation, prepared infant formula was not labeled with the date.

Evidence:
1. There were 6 bottles of prepared infant formula in the infant room refrigerator that were not labeled with the date for the day at the time of the inspection.

Plan of Correction: We will make sure all bottles are dated moving forward.
