A renewal inspection was conducted on April 18, 2024 from 3:00 P.M.-4:30 P.M. There were 25 children present, ranging in ages from five to ten years of age, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.011-F Description: Based on a review of required postings, the center failed to ensure that the findings from the last inspection were posted.
Evidence:
1. The acknowledgment form from the inspection on 01/30/2024 was posted.
2. The inspection report was emailed to the licensee on 02/05/2024.Plan of Correction: We will post the updated report upon receipt.
Standard #: 8VAC20-770-60-C-2 Description: Based on review of staff records, the center failed to ensure that a completed central registry finding was completed within 30 days of employment.
Evidence:
1. Staff #2's date of hire was 12/12/2023. There was not a completed central registry in the record.
2. Administration verified there was not a completed central registry in the file.Plan of Correction: Will email CRS point of contact to check on the pending status of the staff members CRS check.
Standard #: 8VAC20-780-40-G Description: Based on review of children records, the center did not meet the proof of child identity and age requirements as stated in 22.1-289.049 of the Code of Virginia.
Evidence:
1. Child #3's first date of attendance was 11/07/2023. There was not documentation in the record of viewing proof of the child's identity within 7 business days of the child's start date.
2. Administration verified that there was no documenting verification of proof birth for Child #3.Plan of Correction: Will contact the family and request proof of birth.
Standard #: 8VAC20-780-60-A Description: Based on a review of children records, the center failed to ensure all required information was in the children records.
1. Child #4's record did not contain a full address for both of emergency contacts listed.
2. Child #3's file did not contain the work phone number for both parents.Plan of Correction: Family to be contacted to obtain additional information for the file.
