Technical Assistance:
Discussion on the name change from Prestige Preschool Academy to Everbrook Academy.

DHO training options were discussed. The website for the Community College Workforce Alliance ccwatraining.org/training-for-individuals/professional-development/child-care was shared. A course entitled "Health Screening for Child Care Professionals can be found on this website.

Discussion of the 90-calendar days time frame for new staff in regard to the completion of the 10-hour Virginia Pre-Service Training.

Comments:
This renewal inspection was initiated by licensing staff and concluded on 04/25/2024, between the hours of 10:30 am to 2:30 pm. There were 49 children present, ranging from infant to preschool-age, with 8 staff supervising, across 5 separate classrooms. The children were engaged in the following: napping, lunch, hand washing, bathroom breaks, center activities within their classroom, picture day, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children files, and 14 staff files were reviewed.

Information gathered during the inspection determined that there were (6) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review, 1 of 14 staff files indicated that each staff member did not submit documentation of a negative tuberculosis screening (TB) at the time of employment (prior to coming into contact with children), and that this documentation (signed by a physician, physician's designee, or an official of the local health department) had been completed within the last 30 calendar days of their date of employment.

Evidence -
1. On the date of inspection (04/25/2024), it was determined that the start date for Staff #11 was listed as - 04/23/2024.
2. The TB documentation on file for Staff #11 was dated - 04/24/2024.

Plan of Correction: The school management will ensure that the staff file checklist is completed in full before staff starts so that TB documentation is dated before employees start date. The TB test for staff #11 was obtained prior to them coming into contact with children as they were training on their first day and were not in classrooms.

Standard #: 8VAC20-780-60-A
Description: Based on review, 1 of 5 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (04/25/2024), the documentation of viewing proof of Child #5's identity and age was not available.
2. The start date for Child #5 was listed as - 07/10/2023.

Plan of Correction: The Birth Certificate was viewed by Director for Child #5 on 4/25/24 and documentation of that was entered in the child's file. The child file checklist was reviewed by all members of management and we will ensure that all children have proof of identity and age before enrollment.

Standard #: 8VAC20-780-240-A
Description: Based on review, 2 of 14 staff files indicated that the Virginia Department of Education-sponsored orientation course was not completed within 90 calendar days of employment.

Evidence -
On the date of inspection (04/25/2024), the following staff did not have a certificate of completion available for this 10-hour training:
1. Staff #10 (start date - 12/11/2023)
2. Staff #13 (start date - 12/27/2023)

Plan of Correction: Staff #10 & #13 have completed the Virginia Department of Education sponsored orientation course and the certificate has been added to their files. Moving forward, the school will ensure the Orientation training occurs within the 90-day timeframe.

Standard #: 8VAC20-780-245-L
Description: Based on review, there was not at least one staff member on duty who had obtained within the last three years instruction in performing the daily health observation (DHO) of children.

Evidence -
1. On the date of inspection (04/25/2024), the most recent DHO certificate on file at the center was dated - 01/30/2020.
2. This DHO certificate was held by Staff # 2.

Plan of Correction: On 4/30/24 the Director completed the DHO training from the Community College Workforce Alliance website that was shared during the licensing visit.

Standard #: 8VAC20-780-260-A
Description: Based on interview, the center was unable to provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

Evidence -
1. The Center Director stated (during an interview conducted on the date of inspection - 04/25/2024) that the last know fire inspection conducted by the Loudoun County Fire Marshal's office was from November 2022.
2. Center Director stated that she did not realize that an annual fire inspection must now be scheduled through the Fire Marshal's office in advance. She also stated that a fire company had recently come by to review where the safety key was kept in case of an emergency when the center was closed.

Plan of Correction: The school submitted an application on 4/30/24 to the Loudoun County Fire Marshal Office to have a renewal inspection completed as soon as possible. The school has set a calendar reminder to apply for the renewal annually.

Standard #: 8VAC20-780-500-B
Description: Based on observation, a non-absorbent surface for diapering or changing was not in use for children younger than three years.

Evidence -
1. On the date of inspection - 04/25/2024, within both the Early Preschool Room and Toddler Room, the changing pad had its surface worn away displaying the padding underneath.
2. The pads in both rooms were removed.

Plan of Correction: The changing pads were immediately removed, new pads were ordered on 4/24/24, and new pads were installed on 4/30/24. Teachers have been trained to look for tears in the pads and torn pads will be removed and replaced immediately.
