Comments:
An unannounced monitoring inspection took place on May 1, 2024, between the hours of 11:45am and 3:29pm. There were four (4) classrooms observed with a total of 48 children present with 12 staff supervising within the supervision guidelines. The children were observed playing with various manipulatives, outside activities, eating lunch, diapering, handwashing, and resting or sleeping on their mats. Positive interactions were observed between the staff and children. A complete inspection of the physical plant, children and staff records, evacuation drill logs, nutrition, medication, required posted documents, first aid kit, and emergency procedures were observed during this inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Violations:
Standard #: 8VAC20-780-130-E
Description: Based on record review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record for Child #2 (age 15 months) did not have documentation of additional immunizations every 6 months. The last documentation of immunizations was dated 08/03/2023.

Plan of Correction: I will contact the parent today and inform them I must have an updated immunization record on or before May 28, 2024.

Standard #: 8VAC20-780-40-M
Description: Based on observation and interview, the center did not maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions where children are present.
Evidence:
1. The Infant Classroom did not have a written list of children's allergies.
2. Staff #5 confirmed the classroom did not have a written list of children's allergies, but stated there was a child in the classroom with allergies.

Plan of Correction: I will get the allergy list posted immediately.
Violation was corrected during the inspection.

Standard #: 8VAC20-780-500-A
Description: Based on observation, staff did not wash their hands with soap and running water before and after a diaper change.
Evidence: Staff #5 did not wash her hands before and after changing the diaper for Child #3.

Plan of Correction: I will retrain all my staff on proper handwashing today and we will include additional training at our May 30th staff meeting.

Standard #: 8VAC20-780-500-B
Description: Based on observation, disposable towels were not used in the diapering area and the diapering surface was not sanitized after each use.
Evidence:
1. Staff #5 (Infant Classroom) used a green cloth to clean and sanitize the diapering surface.
2. Staff #5 (Infant Classroom) did not properly sanitize the diapering surface by allowing the surface to air dry for a minimum of two minutes or according to the disinfectant solution instructions.

Plan of Correction: I will remove the cloth and speak to the staff immediately. Also, we will retrain everyone at our May 30th staff meeting. Violation was corrected during inspection.

Standard #: 8VAC20-780-520-C
Description: Based on observation, diaper ointments and creams were accessible to children.
Evidence: The Infant classroom had a container of diaper ointments and creams stored inside the changing table that was accessible to children.

Plan of Correction: I will move the container immediately and remind the staff the ointments and creams must be inaccessible to children. Also, I will review the standards regarding over-the-counter products at our May 30th staff meeting.

Violation was corrected during inspection.
