Comments:
Today an unannounced monitoring visit was conducted with the Center Summer Camp Director. This visit took place in the morning between 8:45am-10:30 there were 8 children present with a total of 1 staff within the supervision guidelines. The children were observed arriving to morning summer camp, having morning snack, daily free play activities. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed. Standard #: 22VAC15-51-40-C-1
Description: Based on a review of the staff records, one record did not include documentation of an original Criminal record and central registry check before the end of 30 days of employment. Evidence: Staff #1's record did not include documentation of the results of the criminal record check and central registry check who began employment 3/16.

Plan of Correction: Center Camp Director stated that he will notify both agencies to find out why they havent received the forms back & wll verified with credit card company that the credit card was charged.

Standard #: 22VAC40-185-160-C
Description: Based on review, 1 of 2 staff files reviewed indicated that it has been more than two years from the date of the most recent screening or testing, (or more frequently as if recommended by a licensed physician or the local health department), staff members did not obtain and submit the results of a follow-up tuberculosis (TB) screening as stated in subsection B of this section. Evidence - 1. Staff #2's most recent TB documentation on file was dated - 06/16.

Plan of Correction: Staff will have an updated TB screening completed by the end of the week.

Standard #: 22VAC40-185-70-A
Description: Based on review, 1 of 2 staff files reviewed indicated that not all of the required documentation was on file for each employee. Evidence - 1. Staff #1's record did not include documentation that the references were checked & verified prior to employment.

Plan of Correction: Center Camp Director stated that he will notify president of operations and request the documentation for the record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, an area inside had two separate maintenance issues with regards to safety and operable condition. Evidence: 1. On the date of inspection (7/19/16), within the multi-purpose/snack room, three holes were present from what was previously an additional coat rack next to the existing one.

Plan of Correction: Center Director stated that they will have the holes fixed before the end of the week.
