Comments:
On 11/20/2020 a virtual Subsidy Health and Safety inspection was conducted. Due to the COVID 19 pandemic the inspector was not on site. This inspection was conducted via mobile video application, email and phone correspondence. During this inspection there were 3 children present observed playing freely supervised by the vendor . Emergency supplies, procedures and drills were reviewed. The home and facilities were inspected via video mobile application. In addition, Child, provider and household member records were reviewed. Violations are indicated in the violation notice.

Standard #: 22VAC40-665-160-B
Description: Based on request and conversation with vendor it was determined that daily attendance was not maintained.

Evidence: During inspection vendor stated that they did not have daily attendance due to running out of standard form for documentation.

Plan of Correction: I didn't know the form was not needed I will keep attendance daily.
