Comments:
An unannounced complaint inspection was initiated on 3/8/18, with return dates to the center of 3/16/18 and 4/16/18. Interviews were conducted with children and staff relating to the allegation of inappropriate supervision. A review of staff and children's records was also conducted during the inspections. The preponderance of the evidence gathered was sufficient to complete a Violation Notice. Please complete the columns for the "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 calendar days of receipt. Description: Based on observation during the complaint inspection on 3/8/18 the center failed to maintain ratios during nap time. Evidence: 1. On 3/8/18 during nap time the LI witnessed eight children in the infant area sleeping. There were two-seven month old and six children two to five years old with one staff. The center needed one more staff person in this grouping during nap time.
