Comments:
Today an unannounced monitoring visit was conducted with the Center Director and Summer Camp Director. This visit took place in the morning between 10:00am-12. There were 6 children with 1 staff within the supervision guidelines. The children were observed having morning snack, arts and craft, free play in the gym and in transition for afternoon dismissal. Positive interactions were observed during this inspection. A complete inspection of the physical plant, staff records, attendance, fire drill log, and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of the children records, one record did not include a complete physical exam. Evidence: Child #2's record, did not include a copy of the physical exam.

Plan of Correction: Director stated that the family will be notified today and will request a copy.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the physical plant, the following areas of the center were found to be not in a clean, safe and operable condition. Evidence: Small holes were found along the wall closest to the window and along the main wall to the outdoor area. Chipped paint was found along the wall in the room used for summer camp.

Plan of Correction: Director stated that they will have the walls repainted and holes fixed as soon as possible.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the physical plant, cleaning agents were found unlocked and accessible to children. Evidence: In the room used for summer camp and gym, cleaning agents were found in a unlocked storage closet and cabinet with the safety latch unsecured. In the gym, disinfectants were found along the rail unlocked and accessible to children.

Plan of Correction: Director stated that all doors are securely locked and cleaning agents will be stored at all times and out of reach of children.

Standard #: 22VAC40-185-280-G
Description: Based on observation of the physical plant, a substitute container attached to a mop was found unlabeled. Evidence: In the summer camp room, a mop was found in a storage closet with a container attached that was not clearly labeled to indicate its content.

Plan of Correction: Director stated that the mop container will be labeled today.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on a review of the staff records, one record did not include documentation of an original Central registry check before the end of 30 days of employment. Evidence: Staff #1's record did not include documentation of the results of the central registry check who began employment 3/16.

Plan of Correction: Director stated that they mailed off the Cps check however they have not received it back yet. The check has not been cashed.
