This inspection was conducted by staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the home was not conducted.
A unannounced subsidy inspection was conducted at 3:00pm. The vendor was contacted by phone to initiate the inspection, and provided a list of items required to complete the inspection. A Spanish translator assisted with communications between vendor and inspector. There were 3 children present = 8 points with the vendor. Ratios were in compliance. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Standard #: 22VAC40-665-170-B Description: Based on records reviewed, it was determined that 1 of 1 children's records did not include required information. Evidence: Child # 13 - There was name, address, and phone number for an emergency contact person. The space was left blank. Plan of Correction: The vendor will have the parent write this information on the form.
Standard #: 22VAC40-665-230-H Description: Based on interview conducted, it was determined that the vendor had not completed a required training by the 3/1/19 due date. Plan of Correction: The inspector will provide registration information, and the provider will complete the training.
