22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:
Discussed background checks, and staff records.

An unannounced monitoring inspection was conducted today from 1:00pm-3:15pm. There were 2 children (4-5yrs.) directly supervised by 1 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating snack, coloring, doing crafts, and participating in organized gym activities during afternoon camp. Positive interactions were observed during the inspection. Hand-washing procedures were also observed. There were no medications on site today. There was an abundant supply of materials for the children. The center was clean and organized. Standard #: 22VAC40-185-160-A
Description: Based on record review, the center did not have documentation of a tuberculosis (TB) test or screening for a staff member within 21 days of hire. Evidence: Staff#4's record did not contain TB test/screening documentation.

Plan of Correction: The staff member is going today to get the TB test/screening done.

Standard #: 22VAC40-185-160-C
Description: Based on staff record review, the center did not obtain updated tuberculosis (TB) test or screening for each staff member every two years. Evidence: Staff#3 has a TB test on file that expired on 3/8/2018.

Plan of Correction: The staff member will get the TB test/screening done.

Standard #: 22VAC40-185-60-A
Description: Based on record record review, the center did not maintain complete children's records. Evidence: Child#1's record is missing the father's work telephone number. Child#2's record is missing an address for 1 of 2 required emergency contacts. Child#4's record is missing the address for both emergency contact. Child#5 is missing the mother's work telephone number, and the address for 1 of 2 required emergency contacts.

Plan of Correction: We will request the information from the parents.

Standard #: 22VAC40-185-270-A
Description: Based on observations, areas and equipment of the center were not maintained in a safe and operable condition. Evidence: In the gym area, on the left side towards the back of the room a section of the plastic baseboard lining the wall was observed coming loose from the wall creating a possible tripping hazard.

Plan of Correction: We will fix the baseboard.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and staff interview, the center did not obtain a Central Registry report for a staff member within 30 days of employment. Evidence: Staff#4's record did not contain a Central Registry report. Staff#4 was hired on 8/20/2017.

Plan of Correction: we will obtain the documentation needed.
