Standard #: VENDSUB-000-034-C Description: Based on the observation of the feeding of infants in the infant room, the staff failed to follow the procedures for feeding an infant who can not yet hold their own bottle or an infant in their sleep area. Evidence: The staff had the infant in a moving infant seat and propped the bottle up by stuffing a blanket under the bottle and tugged along the infant to keep the bottle in place. A second infant was laying in their crib drinking a bottle. Plan of Correction: I will go over the procedures of how we feed babies again.
Standard #: VENDSUB-000-006-B Description: Based on the review of 7 children's records, it was determined that the center failed to get all components to keep the children's files complete. Evidence: 1 child, child D, is missing the address for the emergency contact. Plan of Correction: I will ask the parent for the information
Standard #: VENDSUB-000-015-A Description: Based on the review of the physical plant inside and out it was determined that the center failed to keep some parts of the playground in a clean and operable condition. Evidence: The playground has some lawn furniture that is in pieces and stored in tall grass by the shed and a tire swing that has the chains covered with duct tape. The tape is worn and coming off in threads. Plan of Correction: I will notify the building and grounds and we will get it fixed.
