Comments:
An unannounced renewal inspection was conducted today from 9:30am-12:00pm. At the time of inspection, 123 school age children were directly supervised by 15 staff and ratios were in compliance. The sample size consisted of 5 children records and 5 staff records. Children were observed participating in group play, playing knockout in the gym, and making galaxy slime. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in medication administration, CPR and First Aid, as well as daily health observation training. Description: Based on review of injury records, the center did not maintain a complete written record of a child's minor injury. Evidence: An injury report for child #1 did not have the date and time the parent was notified of the incident or two signatures from staff and/or parent.

Plan of Correction: The new form for injury reports will be used to ensure all required items are met.
