Comments:
An unannounced renewal inspection was conducted today from 11:30am-1:00pm. At the time of inspection, 104 school age children were directly supervised by 14 staff and ratios were in compliance. The sample size consisted of 5 children records and 5 staff records. Children were observed participating in group play, having lunch (packed lunch and lunch provided by the Cafe), and playing a game of sharks and minnows. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in medication administration, CPR and First Aid, as well as daily health observation training. Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance. Evidence: The record for child #1 (start date: June 18, 2018) did not have documentation of a physical examination.

Plan of Correction: I will ask the parents to provide an updated physical.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children?s records, the center did not maintain complete children?s records. Evidence: The records for Child #2, #3, and #4 did not contain names, addresses, and phone numbers for two emergency contacts.

Plan of Correction: Staff will ask parents for complete emergency contacts at pick-up today.
