This annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic.
Preliminary data was obtained from the vendor on 04/28/20.
An annual subsidy inspection was initiated on 04/29/20, at which time the vendor was contacted by telephone to initiate the inspection, and concluded on 04/30/20. The inspector emailed the vendor a list of items required to complete the inspection.
The vendor reported there were 3 children in care and 0 children living in the home under the age of 12 present with the vendor. The inspector reviewed 3 children?s records and 1 adult/household member record submitted by the vendor and health and safety regulations.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Standard #: 22VAC40-665-170-B Description: Based on record review and interview, the vendor failed to provide all required information for one out of three children's records.
Evidence:
1. The vendor did not provide a written authorization for emergency medical care and sick child policy signed by a parent for child 3.
2. The vendor stated she thought she had the paperwork but when she was asked to provide it within 24 hours she did not provide the information or respond to the request.Plan of Correction: The licensing inspector (LI) provided information to the vendor on how to obtain the documents for the parent to sign. The vendor will have the parent sign the documents and forward the signed documents for the LI to review and the vendor will then file the documents. In the future any new children's parents will be required to complete the documents upon registration.
