This inspection was conducted by staff using an alternative remote protocol, including phone interview, and records review. An on-site inspection was not conducted at this time.
An unannounced subsidy inspection was conducted at 10:03am. The vendor was contacted by phone to initiate the inspection. There were 2 children present = 5 points with the vendor. Ratios were in compliance at the time of inspection.
Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Standard #: 22VAC40-665-230-H Description: Based on information received, it was determined that an annual training class has not been completed in the last 12 months. Evidence: There was no documentation that the vendor has completed the Virginia Health + Safety Training class, which is required every year for subsidy vendors. Plan of Correction: The vendor will complete the class in the next 30-days and send the inspector a copy of the training certificate.
