Technical Assistance:
1. Written hard-copies of daily attendance for children is required for the subsidy program. Inspector will send model form.
2. All medications in the home are to be kept in a locked location when children are present.
3. Inspector will send emergency drill record model form, and training information.

Comments:
An unannounced subsidy inspection was conducted at 10:22am. The vendor. Adult #1, and a visiting relative were present in the home. The vendor speaks English as a second language, and while the vendor was able to communicate some on her own, Adult #1 also helped with communications. There were no children present at the time of the inspection, and for the most part supplies and equipment were put away, but present in the home. Children bring their own food daily. Activities include walks in the neighborhood and to a nearby playground. No transportation provided. The current County license was not posted in the home (one that expired in 2021 was posted), but post-inspection the inspector verified the license to be current. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Violations:
Standard #: 22VAC40-665-230-H
Description: Based on records reviewed and interview conducted, it was determined that the vendor has not completed a required annual training class. Evidence: There was no documentation that the provider had completed the Virginia Health + Safety Training class in the previous 12 months. Adult #1 stated that he did not believe the vendor took the course.

Plan of Correction: The vendor will complete the course in the next 90-days, file the training certificate in her record.

Standard #: 8VAC20-790-180-B
Description: Based on information requested, it was determined that a daily attendance that documents the arrival and departure of each child in care as it occurs has not been maintained. Evidence: The inspector requested a copy of the attendance records for each child, and none were received.

Plan of Correction: A written daily attendance will be kept for each child. These records will be kept for 5 years after care is terminated.

Standard #: 8VAC20-790-200-2-a
Description: Based on information requested, it was determined that the provider and an adult household member do not have a current criminal background check/fingerprinting results. Evidence: On 4/22/22, the inspector requested the fingerprinting results for the vendor and Adult #1 and was told the County held these documents. On 4/22/22, the inspector contacted the County and was advised that this was not the practice, and that the vendor is to maintain all background checks. The inspector advised the vendor of such, and requested background checks be submitted for review by 5:00pm on 4/26/22. To-date, the fingerprinting results remain outstanding and have not been submitted by the vendor.

Plan of Correction: 10-days will be given for the each individual to complete and submit the criminal record check/fingerprinting results to the inspector.

Standard #: 8VAC20-790-230-A
Description: Based on records reviewed, it was determined that 1 of 2 children's records did not include documentation of immunizations before the child was allowed to attend. Evidence: Child #2 - Began attending in September 2020, and to-date there is no documentation of immunizations.

Plan of Correction: The parent will provide current immunization records.
