Technical Assistance:
Please make sure that medications and supplies for your own medical needs are kept in a different area from your first aid kit. Used hypodermic needles should be discarded in a sharps container or a trashcan and supplies not needed in the first aid kit should be stored in an area away from the area used for care.

Comments:
An unannounced SHSI inspection was conducted on 5/20/22. The vendor is present with her aunt. The child she has in care is sick and absent today.
The vendor has supplies for the children and space for play. Large green space outside.
A violation notice was given to the vendor along with this inspection.

Description: Based on the physical plant review the vendor did not store cleaners out of reach of children.
Evidence: The vendor has gallon bleach bottles stored under the sink in the bathroom, under a plastic table skirt attached to the sink.

Plan of Correction: I will make sure I store cleaners out of reach

Standard #: 22VAC40-665-390-B-1
Description: Based on the review of the vendors non-medical supplies, the flashlight is not in working condition.
Evidence: The flashlight needs batteries

Plan of Correction: I will get batteries for the flashlight

Standard #: 22VAC40-665-390-B-2
Description: Based on the review of the non-medical emergency supplies, the vendor failed to have a battery operated radio.
Evidence: The vendor does not have a battery operated radio.

Plan of Correction: I will buy a battery operated radio

Standard #: 8VAC20-790-180-B
Description: Based on the review of the vendors paperwork the vendor failed to keep a record of children in attendance documenting the arrival and departure.
Evidence: According to the vendor she has not taken attendance since November 2021 and could not locate the attendance sheets.

Plan of Correction: I will make sure that I keep up to date attendance
