An unannounced monitoring inspection was conducted today 12/7/2018 from 10:30 am- 1:30 pm. Child to teacher ratios and sight and sound supervision were in compliance. Children were observed participating in reading and writing activities, free play while in extended day program and preparing for holiday activities. A tour of 2's classroom diaper changing and rest room areas was also conducted. Hand washing and self care routines were discussed. A sample of staff records and written disclosure of information and policies for parents were also reviewed. Standard #: 63.2(17)-1720.1-B-2 Description: Based upon records reviewed, required background search findings were not completed as required. Evidence: Staff A did not have determination letter on record for the fingerprint - based criminal history check. ( appointment was 10/2018) Plan of Correction: Director was following up with OBI to locate determination letter within 10 days of inspection.
