An unannounced monitoring inspection was conducted today, February 5, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 12:00pm and concluded at approximately 3:30pm. All requirements were reviewed including administrative records, all areas in the center used by children, and outdoor areas. See attached notice for violations found during this inspection. Standard #: 22VAC40-191-40-D-2 Description: Based on the inspection of administrative records, the required central registry background check was not obtained. Evidence: During the inspection of five staff records, it was discovered staff #3 did not have a completed central registry background check in the record. The central registry background check is required by the end of the staff's 30th day of employment with documentation maintained for review. Plan of Correction: Per director: I will check with the agency to receive a duplicate for the record.
