Comments:
An unannounced subsidy health and safety inspection was conducted on October 6 2022 between 9:00am and 10:00am. There was 1 child in care of the vendor. There were books and toys for the child to play with. Provider training, household background searches, the child's record, emergency drills, and the physical space was observed and inspected. Description: Based on record review, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: The vendor stated that she does not keep a daily attendance record.

Description: Based on record review, the vendor did not keep complete written agreements between the vendor and the parent.

Evidence: Child #1 record did not contain completed Emergency Preparedness Plan information, the Authorization for Emergency Medical Treatment Document, and the Child Care Agreement form.
