Discussed with the vendor, physical plant, records, emergency preparedness plan and activities, transportation requirements.
- Comments:

Conducted an unannounced annual subsidy inspection on 10/14/2022 from 12:20pm to 1:25pm. Observed 1 child (4 points) with the vendor. The child was napping during the inspection.
The home, emergency drills and emergency supplies were inspected. Children's information and vendor records were also reviewed.
Information gathered during the inspection determined areas of non-compliance violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-420-C Description: Based on observation and interview, the vendor did not have a 911 or local dial number for police, fire, and emergency medical services and the number of the regional poison control center posted in a visible and conspicuous place.
Evidence:
