8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

An unannounced annual Unlicensed Subsidy Health and Safety (USHSI) inspection was conducted on 11/01/2022 between 11:15 am and 11:50 am. There were no children present during the inspection.
The home, staff / children/ household member?s records, emergency supplies, and policies were inspected/reviewed to ensure compliance with the health and safety regulations.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-790-190-B Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain documentation that the child's identity has been verified.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-790-210-A Description: Based on record review and interview with the vendor, it could not be determined if the vendor had been evaluated by a health professional and be issued a statement that is determined to be free of communicable tuberculosis (TB).
Evidence:

Standard #: 8VAC20-790-230-A Description: Based on record review, the vendor did obtain documentation that the child has been immunized, before the child may attend the family day home.

Standard #: 8VAC20-790-410-B-2 Description: Based on review of emergency supplies and interview with the vendor, one working, battery operated radio was not available.
Evidence:
1) On the date of inspection, a battery operated radio was available in the fdh, however, the batteries it did not have batteries, making it non-working. The vendor stated that she did not have additional batteries.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-790-460-C Description: Based on interview with the provider, it could not be determined that any vehicle used by the vendor for the transportation of children meet all the requirements.
Evidence:
1) On the date of inspection, the vendor stated that on certain days she transports Child #1 from school to the fdh. The vendor could not provide any documentation to demonstrate that the vehicle meets all the requirements. The vehicle was not in the premises so the licensing inspector could not perform a visual inspection.Plan of Correction: Not available online. Contact Inspector for more information.
