This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview, and a virtual tour of the program.
A monitoring inspection was initiated on 8/2/2021 and concluded on 8/5/2021. The Director was contacted by telephone and a virtual inspection was conducted at 11:35am. There were 54 children present, ranging in ages from 5 years to 11 years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed.

Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, 1 out of 4 children's records did not have proof of identification and age on file.
Evidence:
1. Child #3 (doe: 6/21/2021) did not have a valid proof of identification and age on file.Plan of Correction: We will ask the parents for this tomorrow.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on a review of staff records, 1 out of 5 staff did not submit to fingerprinting prior to the first day of employment.
Evidence:
1. Staff #5 (doh: 6/9/2021) had fingerprints results on file that were dated 6/13/2021.Plan of Correction: Fingerprints will be completed prior to employment moving forward.
Standard #: 63.2(17)-1720.1-B-4 Description: Based on a review of staff records, 2 out of 4 staff records did not have a copy of the results of a criminal history record information check or a sex offender registry check from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #2 (doh: 6/9/2021) did not have a sex offender check on file during the inspection for state #1 and state #2 in which the individual has resided in the preceding five years.
2. Staff #2 did not have a criminal history check on file during the inspection for state #1 in which the individual has resided in the preceding five years.
3. Staff #3 did not have a sex offender check on file during the inspection for state #3 in which the individual has resided in the preceding five years.Plan of Correction: These will be completed immediately.
