Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A renewal inspection was initiated on 8/16/2021 and concluded on 8/19/2021. The site supervisor was contacted by telephone and a virtual inspection was conducted. There were 55 children present, ranging in ages from 5 years to 12 years old, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22VAC40-185-70-A
Description:	Based on a review of staff records, 1 out of 5 staff references as to character and reputation as well as competency were checked not checked before employment. Evidence: 1. Staff #1 (doh: 6/15/2021) had two references on file that were completed after employment on 7/15/2021 and 8/2/2021.
Plan of Correction:	Moving forward, all references will be obtained prior to employment.
Standard #:	22VAC40-191-60-B
Description:	Based on a review of staff records, 1 out of 5 staff were employed before obtaining a completed sworn statement or affirmation. Evidence: 1. Staff # 5 (doh: 8/12/2021) had a sworn statement on file that was dated 8/16/2021.
Plan of Correction:	Sworn statements will be signed by the first day of employment moving forward.
Standard #:	63.2(17)-1720.1-B-4
Description:	Based on a review of staff records, the facility did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years for 2 out of 5 staff members. Evidence: 1. Staff #2 (doh: 7/12/2021) did not have an out of state central registry background check, sex offender background check, or criminal history background check for state #1 in which the employee resided in the preceding five years. 2. Staff #3 (doh: 6/16/2021) did not have an out of state sex offender background check and an out of state criminal history background check for state #2 in which the employee resided in the preceding five years.
Plan of Correction:	The required out of state background checks will be obtained within the next 10 business days.
