Comments:
An unannounced renewal inspection was conducted 06/13/2022. There were 120 children, ages 4 years-13 years, 11 staff providing direct care, and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies and nutrition. The children were observed during breakfast, in free choice activities and departing for a fieldtrip. A total of 10 children?s records, 10 staff records, 3 medications, and 6 applicant/agent records were reviewed during the inspection. The inspector discussed the following with staff: fieldtrips, medication and staff training. The inspector arrived at 8:15 am and departed at 11:30 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review and discussion with staff, the center failed to ensure that every employee at a child day center shall undergo a background check in accordance with the Code of Virginia prior to employment and every five years thereafter.

Evidence:
1. The record for Staff 2 contained a documented hire date of 02/11/2013. The most recent search of the central registry was dated 05/27/2015; there was not a repeat search of the central registry obtained in 2020.
2. The record for Staff 3 contained a documented hire date of 03/17/2017. The most recent search of the central registry was dated 05/27/2017; there was not a repeat search of the central registry obtained in 2022.

Plan of Correction: Will send off the new ones today.

Standard #: 8VAC20-780-160-C
Description: Based on record review and discussion with staff, the center failed to ensure that at least every two years from the date of the initial screening or testing, staff member shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence: The most recent tuberculosis test/screening for Staff 2 was dated 09/12/2017; there was not a follow-up test/screening for 2019 or 2021 available for review.

Plan of Correction: Will get it today.

Standard #: 8VAC20-780-70
Description: Based on discussion with staff, the center failed to ensure that the record for each staff shall contain all of the elements as required by the standard.

Evidence: There was no documentation of the name, address and telephone number of a person to be notified in an emergency, which shall be kept at the center, onsite for any of the staff members present during the inspection.
