Comments:
A monitoring inspection was conducted on 06/15/2022. The inspector reviewed compliance in the areas of administration and staffing. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with an administrator during the exit interview.

Violations:
Standard #: 8VAC20-780-70
Description: Based upon review of staff records and staff interview, the facility has not ensured that there is documentation that two or more references as to character and reputation as well as competency were checked prior to employment.
Evidence:
1. The record provided for staff 3 included documentation of only one reference check.
2. Staff 6 acknowledged that there was documentation of only one reference check in the record for staff 3.

Plan of Correction: The facility responded with the following:
In the future, at least two references will be checked and documented prior to employ of new staff.
