8VAC20-790 Subsidy Program Vendor Requirements for Subsidy Providers

Comments:
A Subsidy Health and Safety Inspection was conducted on 1/12/2023 from 12:20pm through 1:15pm. There were 4 children directly supervised by the vendor, including the vendor's child. During the inspection the home, activities and interactions with children, emergency supplies, procedures and drills were inspected. Three children's records, vendor information and household member information was reviewed.

Information gathered determined violations with applicable code or requirements and are listed on the Violation Notice.

Violations:
Standard #: 8VAC20-790-180-B
Description: Based on interview, the vendor does not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: When asked for the daily attendance, the vendor stated that they do not write down attendance.

Plan of Correction: I will start doing it when you give me the form.

Standard #: 8VAC20-790-190-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child #1's record did not contain someone other than the parents to contact in case of an emergency, documentation that the child's identity had been verified or authorization for medical care.
2. Child #2's record did not contain someone other than the parents to contact in case of an emergency, documentation that the child's identity had been verified or authorization for medical care.
3. Child #3's record did not contain the address of their emergency contact and documentation that the child's identity had been verified.

Plan of Correction: I will contact the parents to give me the missing information.

Standard #: 8VAC20-790-250-A
Description: Based on interview, the vendor did not complete Virginia Preservice Training for Child Care Staff sponsored by the Department of Education.
Evidence: When asked for the Virginia Preservice Training certificate, the vendor stated that they didn't do the training.

Plan of Correction: I will do the training after you send me the link.

Standard #: 8VAC20-790-320-B
Description: Based on observation, toddlers spent more than 30 continuous minutes during waking hours confined in a crib, play pen, high chair, or other confining piece of equipment.
Evidence: Child #1 and Child #2 were observed awake, in cribs at the start of the inspection at approximately 12:20pm. The vendor was encouraged to take the children out of the cribs, however the vendor stated that they wanted the children to stay in the cribs during the inspection. The inspection ended at approximately 1:10pm. The children remained awake, in the cribs for the entire duration of the inspection.

Plan of Correction: It's only for the inspection. If I let them out, they'll run all over and touch the papers. I'll take them out when you're done.

Standard #: 8VAC20-790-340-2
Description: Based on observation, enclosure in a small, confined space or any space that the child cannot freely exit themselves was permitted.
Evidence: Child #1 and Child #2 were observed awake, in cribs at approximately 12:20pm. The vendor was encouraged to take the children out of the cribs, however the vendor stated that they wanted the children to stay in the cribs during the inspection. The children were confined and could not free themselves. The inspection ended at approximately 1:10pm. The children remained in the cribs for the entire duration of the inspection.

Plan of Correction: It's only because you're here. I'll take them out when you're done.

Standard #: 8VAC20-790-410-B-2
Description: Based on observation, the vendor did not have one working, battery-operated radio.
Evidence: The vendor's battery-operated radio would not work when tested.

Plan of Correction: I will call my husband and ask them to stop and pick up new batteries on the way home from work.

Standard #: 8VAC20-790-420-C
Description: Based on observation and interview, 911 or local dial number for police, fire, and emergency medical services and the number of the regional poison control center is not posted in a visible and conspicuous place.
Evidence: The contact numbers for local police, fire and emergency services and poison control was not posted within home.

Plan of Correction: I can do that today.
