Comments:
Today an unannounced Monitoring Inspection took place between the hours of 1:50pm and 4:00pm. There was a total of 60 children with 15 staff within the supervision guidelines. The children were observed playing games outside, creating art, conducting dry ice experiments and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 8VAC20-770-60-C-2
Description: Based on a review of staff records, 2 out of 4 staff did not have a central registry finding within 30 days of employment.

Evidence:
1. Staff #2 (doh: 6/8/2022) did not have documentation of the results of a central registry finding on file at the time of the inspection.
2. Staff #3 (doh: 6/8/2022) did not have documentation of the results of a central registry finding on file at the time of the inspection.

Plan of Correction: We will follow-up on these tomorrow. We mailed them back out.

Standard #: 8VAC20-780-160-A-2
Description: Based on a review of staff records, 1 out of 4 staff did not have documentation of a tuberculosis screening within 30 calendar days of the date of employment.

Evidence:
1. Staff #2 (doh: 6/8/2022) had documentation of a tuberculosis screening on file that was dated 3/30/2022 during the time of the inspection.

Plan of Correction: Moving forward, we will get the TB screening within 30 days of the first date of employment.

Standard #: 8VAC20-780-70
Description: Based on a review of staff records, the required staff records were not kept for each staff person.

Evidence:
1. Staff #4 (doh: 6/8/2022) did not have documentation on file of the name, address, and telephone number of a person to be notified in an emergency at the time of the inspection.

Plan of Correction: We will have the staff member give us an emergency contact.
