Technical Assistance:
1.State subsidy inspections are unannounced and are not scheduled ahead of time.

Comments:
An unannounced subsidy health and safety inspection (SHSI) was conducted at 11:30am. Observed 4 children = 11 points with the vendor. A household member was also present and assisting with care as needed during the inspection. Ratios and supervision were incompliance. The children were participating in an online educational program, drawing, playing with toys, and while child ate a snack of pretzels. Lunch today will include: spaghetti, chicken hot dogs, tomatoes, lettuce, carrots, milk and water. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. An area of non-compliance with subsidy requirements and laws reviewed was found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Standard #: 8VAC20-790-190-A
Description: Based on records reviewed, it was determined that 1 of 4 children?s records did not include required information. Evidence: The record for Child #9 did not include verification of the proof of identity, and emergency medical authorization.

Plan of Correction: The vendor said she will put a copy of the birth certificate in the file, and have the parents complete and sign the emergency medical form.

Standard #: 8VAC20-790-390-E
Description: Based on interview conducted and observations made, it was determined that the diaper changing pad has not been cleaned with soap + water and then sanitized with bleach + water after each use. Evidence: The vendor said after each use she sprays the diaper pad with alcohol. The spray bottle was labeled to be isopropyl alcohol.

Plan of Correction: The inspector explained the cleaning + sanitizing process to the vendor and she indicated that she understood.
