1. The inspector provided information about free CPR/First Aid training.
- Comments:

An unannounced subsidy health and safety inspection (SHSI) was conducted at 11:10am. Observed 1 child = 3 points with the vendor. Ratios and supervision were incompliance. A household member was also present. The child was playing with toys and being held by the vendor. Lunch to be served included: beef, rice, broccoli, apples, and milk. The daycare areas of the home were safeguarded appropriately and sufficiently supplied with toys and equipment for the children. An area of non-compliance with subsidy requirements and laws reviewed were found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Standard #: 8VAC20-790-190-A Description: Based on records reviewed, it was determined that 1 of 1 children?s records did not include required information. Evidence: There was no emergency medical authorization signed by the parents for Child #7. Plan of Correction: The inspector provided a blank emergency medical form to the vendor who said she will have parents complete.
