Comments:

A monitoring inspection was conducted on 7/27/202. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Standard #:	22.1-289-036-E
Description:	Based upon review of staff records, the facility has not ensured that there is no person involved in the day-to-day operation of a child day center or who is alone with or supervising one or more children without having first completed a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. Evidence: According to the record provided, staff 4 has lived in North Carolina within the past five years. Results of a sex offender registry check and a child abuse and neglect registry search were not obtained for staff 4 who was on duty with the children during the inspection.
Plan of Correction:	The facility responded with the following: All out-of-state background checks for applicants who have lived in another state within the past five yeas will be obtained prior to the hire of the applicant. Required checks will be obtained for staff 4.
Standard #:	8VAC20-780-320-B
Description:	Based upon observation, the facility has not ensured that restrooms are equipped with soap. Evidence: The girls restroom by the cafeteria was not equipped with soap.
Plan of Correction:	The facility responded with the following: The janitorial staff will be immediately contacted to fill the soap dispensers. Staff will be reminded to notify administration with needed restroom supplies.

Description:	Based upon review of written injury records, the facility has not ensured that injury records include all required written documentation. Evidence: A written injury record dated 7/21/2022 of an injury to child 6 does not have documented the date and time the parent was notified.
Plan of Correction:	The facility responded with the following: All staff will be reminded of the importance of completing all documentation required on written injury records. Injury records will be reviewed to ensure they are complete.
